Insurance Plans

We participate with most insurance plans. Please call our office for information regarding our affiliation with your plan.

Helpful reminder: For HMO participants: Please designate Mesa Family Medical Center as your Primary Care Provider (PCP) or one of our doctors if you plan requires a specific doctor BEFORE your appointment. The person covered by the insurance policy must choose the PCP we cannot do that for you. If our group or one of our doctors is NOT on record with your insurance carrier as your PCP, then your appointment may be delayed or rescheduled until your insurance carrier has the correct physician assigned to you and your family. This is a requirement of your insurance carrier and followed by MFMC contract agreement with your carrier.
